Reason for Stay: Therapy after surgery on her left foot

Details of Experience:
Miss Jackie of Whites Creek, Tennessee, served as a Certified Nurse’s Assistant for 31 years. One morning,  she noticed a black spot on her left foot. She went to see her doctor at Centennial Medical Center. He felt surgery was required to remove the spot. While she was in surgery, they found a blood clot in her foot. When she was recuperating in the hospital, the doctors said she should transition to a rehab center.

Having been at Green Hills Center once before, after back surgery, Miss Jackie was certain where she wanted to go when she prepared to leave the hospital: “They asked me where I wanted to go, and I said ‘Green Hills Center.'” She said she likes the staff and has been really happy since being here.

She arrived late one evening, got settled in her room, and began her therapy early the next morning, and she reiterates that she likes the staff. “I’m a people person—I get along with everyone.”

Miss Jackie’s therapy is going really well, and she is making substantial progress every day. “The OT and the PT are wonderful. Kris is a really good therapist,” she says. He says that she is walking a great deal and attributes her success to her personality—she’s really social.

After her therapy sessions, Miss Jackie can be seen most days sitting outside under the carport, watching the traffic go by. She speaks to everyone who goes in.

Although the staff at Green Hills hasn’t given her a definite discharge date, Miss Jackie is certain it will be very soon. Her goal is to be able to return home and maintain her household. The staff at Green Hills Center wants to make sure she can perform all the duties of living at home before discharging her home permanently.

Miss Jackie has made a significant impact on the community and staff here at Green Hills Center for Rehabilitation and Healing, and the team hopes that she gets home safely and continues to heal and live life to the fullest!

Patient’s Name: Floyd V.
Admission Date: 6/11/4/21
Admitted From: St. Thomas Midtown
Discharge Date: Pending

Details of Experience:
Mr. Floyd, a native Nashvillian, was working for a company called Block by Block, which works to clean, as the name says, downtown Nashville. Over time, Mr. Floyd developed some infections that began with his toe and worked its way to his kidneys, which led to persistent hiccups and gasping for air. When Mr. Floyd realized something serious was going on, he was admitted into St. Thomas Midtown for treatment. Unfortunately, Mr. Floyd was required to get his toe amputated. That was when he searched for a rehabilitation center to return to his normal activities of daily living. Mr. Floyd found Green Hills Center for Rehabilitation and Healing. When he was ready to be discharged from the hospital, he decided to transfer to Green Hills.

Upon arrival at Green Hills Center, Mr. Floyd was blown away by how nice it was. “When I entered Green Hills Center for Rehabilitation and Healing,” Floyd said, “I thought it was a hotel. Everything was just so nice and clean.” He was greeted by the staff and shown to his room, which he thought was very comfortable and neat. He settled in just fine, meeting the staff as they came in to introduce themselves. Before he knew it, it was dinner time! Mr. Floyd was given a hotdog, coleslaw, and baked beans which he enjoys. “My first meal here was delicious!”

Mr. Floyd came to Green Hills for therapy, and therapy was what he got. “The therapy is almost constant,” Mr. Floyd said. “Right now, I need to concentrate on walking for long periods.” He has made great progress so far and loves to work hard with the therapists that he loves. He would like to recognize two therapists, Chantel and Kris, for how hard they make therapy but in a fun and productive way. “They really make a difference.” It’s not only the therapists that have made Mr. Floyd’s stay so nice, but also the nursing staff. He really enjoys Michelle, one of the nursing technicians on the floor, and loves to chat with her when she comes by. “She makes you feel at home,” he said.

Though Mr. Floyd has made significant progress in his therapy since being here. He still has a way to go. There is no discharge date set yet, but it won’t be long until he is home and does the things he loves because of his hard work and determination. He cannot wait to go out and travel again once he is home.

We cannot wait to see how Mr. Floyd does when he goes home and wish him all the best on his future travels!

Miss Judy, unfortunately, suffered a stroke one day and was unconscious for at least two days. “I was in St. Thomas to recover from the stroke and to get checked out physically,” she said. After coming to Green Hills Center for Rehabilitation and Healing, Ms. Judy credits therapy and her own perseverance with the recovery that she has made. Orthopedic Rehabilitation Following Hip Replacement Repair Surgery

Director of Concierge: Garrott Iannelli
Patient’s Name: Mike B.
Patient’s Age: 66 years old
Admission Date: 3/1/21
Admitted From: Skyline Medical Center
Discharge Date: 3/18/21
Reason for Stay: Hip replacement broken

Details of Experience:
After being a sailboat captain for 25 years in Florida, Mr. B sold his sailboat on Lake Erie and retired his seat. Now, he travels around the country in an RV that he calls his “Land Yacht.”  Unfortunately, Mr. B had a double hip replacement, and after about ten years, one of the replacements broke, bringing him to Skyline Medical Center to fix his hip replacement. Mr. B. needed to walk to get around his motorhome and continue working and traveling. To recover, the doctors mentioned that he should plan to go to a skilled nursing facility, which is how he found us here at Green Hills Center.

Upon arrival, Mr. B thought that the facility was nice and clean, significant to him. He enjoyed his room and felt very comfortable right from the get-go. He was thrilled when he saw he had a room to himself because he wants his privacy. He was greeted by many smiling faces through activities and nursing and settled in quickly, ready to work with therapy. The staff treated him well and got him what he needed when he asked.

His primary focus was to walk on the hip through therapy. He was very determined and worked very hard with the therapy team at Green Hills, saying they all did an outstanding job. Mr. B particularly enjoyed his time with Kris, a PTA here. “They were all really good,” Mr. B said. He made a full recovery through hard work and determination and was discharged from therapy, walking around without assistance!

Mr. B. plans to return to work at a KOA campground and continue traveling in his motorhome. He is very appreciative of the work that the staff of Green Hills Center did for him to get him back to where he wants to be. Green Hills is happy to say that he is up walking around again and, on his way, back to work!

Everyone at Green Hills Center wishes him the best on his journeys and is so happy to have been able to help him on his recovery journey!

Reason for Stay: Therapy following infected wound on the leg

Details of Experience:
Miss. Inez, from Madison, Tennessee, served as a nurse for twenty-five years in the Nashville area. Unfortunately, she was diagnosed with degenerative joint disease and MS, which forced her to go onto disability, keeping her home from doing the work she loved.

One day, Miss Inez received a scratch on her right leg that developed into an infection and sent her to St. Thomas Midtown for two separate admissions for IV antibiotics and treatment. Upon going home for the second time, Miss Inez was receiving home physical therapy. Her physical therapist recommended that Miss Inez look into a rehabilitation facility and gave her a list of places to look over. Miss Inez knew of a few people who had been to the Green Hills Center for Rehabilitation and Healing and had heard good things, so she decided to come to Green Hills Center for her own therapy and healing.

Upon arriving from home to the facility, Miss Inez had great experiences meeting the staff and seeing the building for the first time. She noted the community’s cleanliness and how friendly everyone was when interacting with her. She was nervous because this was the first time she had been in a facility like Green Hills Center, but her fears were quickly alleviated. Shortly after settling in, she met her therapy team and began to get to work on her healing journey.

Miss Inez loves her therapy team, saying that she looks forward to having therapy every single day. When they are working with her, they make her feel like she can do anything, but they understand when she feels like she is reaching her limits. Miss Inez feels that she is making progress towards going home.

Miss Inez is very excited to get back home to Madison, where she can open her window blinds and watch nature while she partakes in her favorite activities, such as journaling and reading her bible plan. She is also very excited to get back to her small congregational church, which she misses dearly.

Miss Inez has made a significant impact on the community and the staff here at Green Hills Center for Rehabilitation and Healing, and the team hopes that she gets home safely and continues to heal and live life to the fullest!

Patient’s Age: 48-years-old
Patient’s Name: James K.
Admission Date: 01/14/21
Admitted From: St. Thomas Midtown
Discharge Date: Pending
Reason for Stay: Therapy following COPD and heart issues

Details of Experience:
Mr. James was admitted to Green Hills Center for Rehabilitation and Healing on January 14th, 2021 from St. Thomas Midtown Medical Center for complications with COPD and heart issues. On January 4th, Mr. James was beginning to have some trouble breathing. While working his three jobs at the mission here in Nashville, they decided he needed to go to the hospital. After completing his stay at St. Thomas Midtown, he received a call from Green Hills Center saying that there was a bed available for him to start therapy immediately. He decided it would be a good idea and set out for his stay at Green Hills Center for Rehabilitation and Healing.

When he arrived at the community, Mr. James was unaware of the care Green Hills provided but said that he was pleasantly surprised very soon after his arrival. It was then that he realized that his mom had actually stayed here back in 2016 for a brief period! He was eager to start therapy and didn’t waste any time getting to know his therapy team. At first, they started with simple movement exercises and gradually worked their way up to higher rep counts, even adding weights to the exercises. As therapy continued, Mr. James made lots of friends with the other staff in the community, including the nursing staff. On some afternoons you can even catch him dancing to the jukebox with some of the nursing staff! Mr. James has made some significant progress in the two weeks he has been here and continues to improve with the Green Hills staff backing him up.

When Mr. James is discharged, he is excited to get back to work at the Mission, doing laundry, passing out clothes to the homeless population in Nashville, and helping clean the courtyard so everyone can enjoy it. He makes it his priority to be liked everywhere he goes, and he surely accomplished that goal here at Green Hills Center. We are excited for him to get his life back and look forward to keeping in touch with him while his journey continues.

He has made a lasting impact on the community, just as the community has made a lasting impact on him.

Patient’s Age: 64-years-old
Patient’s Name: Linda C.
Admission Date: 09/01/20
Admitted From: Vanderbilt Medical Center
Discharge Date: Pending
Reason for Stay: Therapy for spinal fusion procedure

Details of Experience:
Linda was admitted to Green Hills Center for Rehabilitation and Healing on September 1, 2020 from Vanderbilt University Medical Center after spinal fusion surgery. A native Tennessean, she had been having lots of back and leg problems, including trouble standing up straight due to severe pain. After going to her primary care doctor, she was referred to a Vanderbilt surgeon specializing in spinal fusion procedures. While there, the doctors and case workers researched and suggested places for her to go for rehabilitation. Her son and husband made the decision for her to come to Green Hills Center.

Upon arrival at the Green Hills Center for Rehabilitation and Healing, Linda was greeted by the warm and friendly receptionist and staff. Linda was not able to participate in a tour but was very happy with her room and all of the staff. She mentioned she didn’t have to ask for anything when she first arrived because the room was prepared with everything she needed. Throughout her stay, Linda shared she felt safe due to the strict COVID-19 protocols the community had in place for both patients and staff.

Linda got right to working hard and her outcomes have been extraordinary. The therapists, she says, help her days pass quickly and she loves them. “I really have started to think about them as family,” she says. “I’ve gotten very close to Ms. Karen and Ms. Vivian.” Linda is now able to sit up in bed and move to the edge of the bed independently for activities and eating meals. She made monumental strides in her recovery. Linda said, “Therapy doesn’t seem like therapy, it seems like two friends just making conversation and having fun.” Upon discharging from Green Hills Center, Linda looks forward to returning home to East Tennessee where she will work inside and outside the house with her husband. Through her therapy regiment, Linda has a lot of movement back to help her with her day to day activities. Not only did the Green Hills staff make an impact in her life, but she also has made a big impact on the team here.

All of the staff wish her a healthy recovery and will continue to support her on her journey every day.

How did this patient hear about Green Hills Rehabilitation and Healing Center? Primary Care Physician recommendation

Details of Experience:
Mr. Lamont has been a Nashville man all his life. Not only has he been a resident in this city for many years, but also strived tirelessly to make this city better throughout his career. He served on a plethora of boards and committees throughout his adult life, working with the Nashville Children’s theater, working on the Nashville Adventure Science Center, and serving as the chairman of the Nashville Parks and Recreation board. Not too long after he decided to retire, Mr. Carter had a stroke at age 69, while at home and was rushed to St. Thomas where doctors were able to help him stabilize. While at St. Thomas, Lamont’s physician recommended he go to a rehab center, and recommended Green Hills Center which is very close to his current home.

Lamont and his brother decided to receive rehabilitation services at Green Hills. Lamont was transported into the facility and greeted by the staff. When Lamont first arrived, he was very underweight, depressed and declining in health. He was very non-compliant with rehabilitation during the first year of his stay. Once he met Hailey, a member of our Rehabilitation team, his outlook began to change. She connected with Lamont and challenged him to put all his remaining energy into learning to walk again. She did push him consistently, but that was just the push that Lamont needed. During his first session in the gym, he was able to walk only four steps.

Lamont worked hard in the gym for over three years. The Rehabilitation team came together and created a focused, customized treatment plan which included helping him get out of bed, setting up his food for every meal for him, and helping him dress himself. Lamont said, “These people come to work every day and are committed to helping me and the other patients get better.” After those first three months of therapy, Lamont is now able to walk over 1,000 steps without taking a break! “I literally got my life back. I went from thinking I was going to pass away in that bed to being able to walk out of the facility to visit friends, get lunch, and walk around on my own!”, Lamont shared.

Mr. Lamont has now been a resident of Green Hills Center for four years and considers this facility his home. He has built great lasting relationships with many of the staff members here and is an integral member of the family that has been formed here. Lamont’s life goal is to be able to walk a full mile without stopping.

He is very grateful to all the staff for helping him reach his goals one at a time.
