Case Study: Green Hills Center for Rehabilitation and Healing Q 4 2025
Concierge Name: Alexander Jones
Patient’s Name: Alphonse P
Patient’s Age: 84 Years Old
Admission Date: 11/21/2025
Admitted From: Vanderbilt University Medical Center
Discharge Date: 12/12/2025
Length of Stay: Three weeks
Reason for Stay: Muscle weakness and severe sciatica

Details of experience:

Alphonse was hospitalized due to muscle weakness in his left side, which limited his mobility and range of motion. Mr. P’s posture suffered greatly as a result, causing him to slouch over. He required assistance with everyday tasks, such as walking, showering, and dressing himself. While admitted to Vanderbilt physicians informed him that he has a major case of sciatica, which plays a major factor in the stiffness and lack of mobility on his left side. His Physician recommended a rehab center that could help him improve. He chose Green Hills from a referral listing given to him by the charge nurse, simply because of our recent Google reviews about our therapy department.

Mr. P arrived at Green Hills by ambulette. Once he arrived, he was immediately greeted by the receptionist, CNA, and unit manager. The Concierge came in shortly after with a welcome gift, introducing himself and his role. During our conversation, I learned that he was a retired doctor. Soon after, the therapy team came and assessed him.

Mr. P’s main focus was to regain strength in his left side so that he could return home. Within two weeks, he went from a painful standing position while walking with assistance to no pain at all, with the ability to get out of bed under his own power and utilize a walker.

Mr. P’s rehabilitation was the quickest I’ve ever witnessed as a concierge. Steve, his PT, credited his knowledge of the body, his understanding of what it took, and his diligent work towards his goal. He was discharged from our community to his residence, where his daughter will be able to assist him along with home health therapy. He enjoyed his short stay with us and all that we, as the Green Hills family, were able to do for him along his journey.
